If your child has not sung yet for the Cantabile director, please do not fill out this form but contact Amanda Elo at amanda.elo@hotmail.com   (For boys 11 and up; girls 12 and up by September 1)

If you have multiple students in the same choir, fill out a separate form for each student.

You can add multiple students by clicking submit another response after pushing submit.
